ONLINE REGISTRATION WILL OPEN JULY 15, 2010

Click the pdf link below to be able to print the registration information and registration form.  You may complete the form (one per person) and mail it to the address provided.  Please do not do on-line registration if you choose to mail in your form.  You will receive a confirmation letter in the mail once your registration form has been processed.



 

REGISTRATION INFORMATION AND GUIDELINES

 

·          Advanced registration is encouraged to avoid long lines and waiting.
·          A separate form must be submitted for each registrant except guests and non-paying spouses.
·          Refer to the Registration Category fee list for appropriate registration fees.    
·          Dentists may only register as dentists.  Dentists may not register as a guest.
·          No person associated with the dental profession may register as a guest.
·          Registration) forms must be postmarked by October 22nd, 2010 to receive the lower fee. 
·          After October 22nd, 2010 you must register on site or online only.
·          On-line pre-registration is available and encouraged via the Holiday Dental Conference website at www.holidaydentalconference.com.  Online registration will be available until November 1st, 2010
·          Registration fees can be paid by cash, personal or company check, VISA,  Mastercard, Discover or American Express.  Please note:  A $30 fee will be charged on all returned checks.


REGISTRATION INSTRUCTIONS

 
1)      Log on to www.holidaydentalconference.com and follow the prompts to HDC registration.  On-line registration will be available July 15th and will close on November 1st 2010  The Holiday Dental Conference Brochures will also be available to download in its entirety by August 1st so that you may print the registration form and mail it to:

2)     
You may make adjustments in your original advance registration only by calling the information number at 704.366.8092 before the October 22, 2010 deadline.  Adjustments may be made after this date on-site.

3)     
Please use only one method of transmission to avoid duplicate records or multiple charges.  If you mail the form, do not use the website.   Please note that telephone registration is not available.

4)     
A separate form must be submitted for each registrant.

5)     
Please register with your business address.  If you are not currently employed, please use the address at which you receive your mail.

6)     
Please register for courses by placing the course code in the appropriate box and the amount in the fee box.
 
      7)      Registration form must be postmarked by October 22, 2010 for pre-registration category fees to apply.  Any registration form dated after October 22nd, will incur the on-site registration fee. On-site registration will be available November 1, 2010 at 7:30 AM.
 

CONFIRMATION OF PRE-REGISTRATION:  Attendees whose are postmarked by October 22nd, will receive written confirmation.


 
CANCELLATION POLICY:  Notice of cancellation must be received in writing.  Should you need to cancel your registration prior to October 31st, 2009, refunds less a $25 handling fee will be made in January, 2010.  Cancellations postmarked after October 31st but prior to November 10th will receive “Holiday Money” refunds less a $25 handling fee in January, 2010 that may be used for the following years conference.  After November 10th, NO refunds or Holiday Money will be issued except in health related emergencies documented by a physician (considered on case-by-case basis). 
 
 
SUBSTITUTION POLICY:  Substitutions are allowed and will be processed free of charge.  To register a substitute, the Holiday Dental Conference must have the following:
1.      Name and category of person not attending.
2.      Name and category of substitute.
3.      Payment of the difference between the substitution category and the original registrant.
 

ATTENDEES WITH DISABILITIES:  If you or your guest(s) have a disability and will require accommodation to fully participate, you must contact the Holiday Dental Conference in writing on or before September 15th, 2009.  The Holiday Dental Conference cannot guarantee that requests received after September 15th, 2009 will be fulfilled.
 

NAME BADGES:  Name badge holders are color coded by attendee category.  Name badges must be visible as you enter the scientific sessions, the exhibit hall and any other Holiday Dental Conference event.  Badges, tickets and conference materials will be available at the Conference Registration Desk during on-site registration hours and will not be mailed.
 


CHILD ATTENDANCE POLICY

 

SCIENTIFIC SESSIONS: In consideration of all Holiday Dental Conference attendees, no children are allowed in the scientific presentations.
 

EXHIBIT HALL:  Due to the presence of large equipment, medical instruments, and numerous other products, an adult must accompany all children under the age of 14 while in the exhibit hall.
